Management of Long-Term Conditions (LTC) present a significant challenge to the individual, the NHS and society. Self-management recognises but seeks to overcome the challenges involved in living with a long-term health condition or caring for someone, young or old with a long-term health condition. This approach to health and well-being is underpinned by the disciplines of health and positive psychology.

 

Our area of expertise is in the development of self management strategies across a range of long-term conditions. We not only have an established track record in the development of resources to support self-management, but also in the delivery of programmes to promote its use through individual and group approaches.

 

The Long-Term Conditions Group has experience in:

  • Training health care professionals and lay people to deliver self-management programmes.
  •  Developing supportive interventions including:

o    Group-based self-management programmes (led by health care professionals and lay tutors)

o    Web-based self-management programmes and Apps

 

The major disciplines we have focused on include:

  • Autistic Spectrum Disorders/ Attention Deficit Hyperactivity Disorder (ASD/ADHD)
  • Cancer
  • Cardiovascular Disease
  • Dementia
  • HIV/AIDS
  • Multiple Conditions
